- Client intake information: details you share in our intake forms prior to a session, which may include information about your wellbeing, lifestyle habits, and areas of focus for your session (for example, sleep, stress, or habit-related goals). This may constitute special category data under UK GDPR (health-related information).

4. Special category data (health-related information)
Where our intake forms ask about matters such as sleep, stress, or habits you wish to address, this may be considered health data under Article 9 of the UK GDPR. We only collect this information with your explicit, freely given consent, and only to the extent necessary to prepare for and deliver your session. You may decline to provide this information, decline specific questions, or withdraw consent at any time by contacting us; this will not affect your ability to book a general enquiry, though it may limit our ability to tailor a session to you.
OZWELL is a complementary, non-medical practice. Our intake forms and sessions are not a substitute for medical diagnosis, treatment, or advice, and any information you share is used solely to inform your bioenergy session.
